China Democracy Party Propose Family Doctor System and Doctor Licensing System

China has established a medical system based on state-owned larger hospitals and a small number of larger private hospitals. This health care system is necessary, but it should not monopoly the medical care market to which it only serve part of demand. Currently, China has such a social problem: On one hand, hundreds of thousands of medical school graduates cannot find a job; On the other hand, more than 1 billion Chinese people cannot afford to see a doctor, and cannot get medical treatment in time. From human-oriented aspect, average people only get really sick and needs the facility of a large hospital a few times in lifetime. By average, 95% of sickness is mild and chronic diseases which can be treated by a family doctor or medical specialist. In order to fulfill the demands of the medical market, and to improve the health care system in China, China Democratic Party propose a family doctors system and doctors licensing system.

Provincial Government should grant license to all medical graduates and medical professionals who pass qualifying examinations. Health management Department of the government should administrate and advise the exercise of individual medical professionals by their citizen numbers or foreign registration numbers. Family doctors must also apply for commercial license in order to run a clinic, therefore they must pay tax like other commercial settings. Family Doctor Clinics should pay a medical malpractice warranty fee to provincial government, to establish a fund for malpractice compensation. Family doctors should be required to have insurance, which will adjust according to the malpractice history of individual doctor. When malpractice exceeds a reasonable level, the insurance company may deny application for insurance. With the above requirements satisfied, doctors can make their own choice to set up their own clinic according to the market demands. Doctors without license or do not qualify for above requirements should be charged for endangering othersó health and life, and legislation should be made to heavily punish such. Insurance fraud should subject the doctor to felony and heavily punishment too.

With the implementation of family doctor system, through market regulation, each citizen and foreign citizen will finally have their own family doctor of their own choice. Because of competition, each family doctor will try to lower the costs and improve the quality of medical service. A family doctor clinic usually needs 1 doctor and several nurses and assistants and a few equipments, therefore the running cost could be lowered to affordable level. Through regular checkup, family doctors could find out serious diseases at an early stage when the disease is easier to be treated. Patientsó information should be entered into a national governmental medical information system, where each patientós medical history was recorded and managed. In case a patient needs emergency treatment, the emergency hospital could get patientós previous medical history after authorization, so that proper treatment can be subscribed. The above stated reasons could greatly lower the medical cost and make medical care affordable for Chinese people.

In the entire health care market, the Government's duty is to establish a system, make the rules and maintain order. If the Governmentós health care budget was allocated to each person, itós neither practical nor meaningful for personnel. The Government's health care budget should be solely used for disaster prevention, as well as emergency and intensive assistance for the poor and uninsured patients. Government should establish an emergency room policy, that treats the problem before charging money. Patients who cannot afford emergency or serious disease treatment could apply for medical assistance to larger hospitals. The latter could apply for funds from the government to cover the medical cost. For those who are not insured, have property or cash, but refuse to pay medical expense, the hospital could sue them on the court and the Judge could decide to forfeit their properties to cover the medical cost. This would make the financially affluent families actively seek insurance. Legislation should be made that the government could monitor the property and cash flow of those who applied for governmental medical assistance to cover their medical treatment. If they manage to make certain amount of money, the government have the rights to bill them for their previous medical cost.

State legislation should require that all hospitals unconditionally accept patients who needs emergency treatment. Patients should provide their citizen ID number to hospitals as long as they can. Patients who run away after emergency treatment without providing citizen ID number, or refuse to provide citizen ID number, or provide false financial information or fabricated citizen ID number, should be charged for fraud and subjected to heavy punishment.
